Recommendation

Each of the forms below has been designed to be easily filled in when opened on your computer. To be certain that you don't lose your work, it is best to open the form or forms you need and immediately select "File" and "Save As" - give the form a unique file name and save to your desktop or a particular file folder on your computer. Close the form and reopen the one you saved on your computer. Inputs that you make on that form can be saved. By following this procedure, you can then easily transfer or email completed forms to the MACE commander at the end of your shift.

If you want to fill in the opened form on your computer and print it that can be done by simply completing the form and selecting "print" on your computer.
